5 to 7-year outcomes support durable fixation and revision readiness

Long-term glenoid fixation remains one of the main challenges in anatomic total shoulder arthroplasty (TSA). At the same time, preserving future revision options is becoming increasingly important as shoulder arthroplasty volumes continue to grow and patient expectations rise.

In this article, we highlight an independent study evaluating the mid-term outcomes of the SMR TT Hybrid Glenoid, the first convertible hybrid glenoid featuring a central Trabecular Titanium peg. With a mean follow-up of 6.5 years and outcomes extending up to seven years, the study provides important evidence on the clinical performance, fixation durability, and revision potential of this implant design.

The findings demonstrated positive outcomes across clinical, radiographic, and survivorship assessments:

  • 0% of patients reported being either “very satisfied” or “satisfied” with their clinical outcome.

  • 76.1% of shoulders achieved an “excellent” result according to the Neer Outcome Rating System.

  • Patients achieved a mean ASES score of 91/100 at final follow-up, reflecting excellent shoulder function and pain relief.

  • Significant improvements in active forward elevation and external rotation were observed compared with pre-operative values.

  • No cases of radiographic glenoid loosening or glenoid osteolysis were reported throughout follow-up.

  • Survivorship of the central Trabecular Titanium peg reached 98.9% at the final radiographic assessment.

  • Stable radiographic fixation was maintained beyond seven years.

  • In cases requiring revision, successful conversion to reverse TSA was performed using the well-fixed central Trabecular Titanium peg as the foundation for the reverse construct, preserving glenoid bone stock.

These findings are particularly relevant because they support the dual role of the Trabecular Titanium peg: providing durable biologic fixation during primary TSA while maintaining a stable platform should conversion to reverse arthroplasty become necessary in the future.

Clinical takeaway

This first mid-term report on the SMR TT Hybrid Glenoid demonstrated excellent clinical results, high patient satisfaction, durable fixation, and remarkable implant survivorship up to seven years after surgery. The absence of glenoid loosening, together with the ability to successfully convert to reverse TSA while preserving bone stock, supports the SMR TT Hybrid Glenoid as a reliable solution for both long-term fixation and revision readiness.
